This program was designed
to help Veterans and surviving spouses receive the
financial support that is necessary to ensure on-going
care. Residents receiving care in assisted living
communities may qualify for this program. Must have 90
days in active duty with at least one day during war
time. Medical diagnosis must be such that

Thorough: Follow up visits
and calls for thirty days to facilitate the transition. We can continue to act as a liaison
after the initial thirty days, to families at a distance, monitoring care and
coordinating additional services, making sure things are going well. This
service will be provided by one of our geriatric care mangers for a
reasonable fee (sliding scale).
